i want so much for this game in the future

- id like to see normal operations as well..
for example: hip operations, cancer, eyecorrection, hair-removal, boobyimplants,
like a realism mode

to keep it funny also the original should stay and add the following
- boobyimplants removal
- old people stick removal
- Dungoen Keeper and other well known figures from games that come for special treatments that have to be researched first. for a bonus like an statue outside of yourself posing.
- custom avatars for all doctors and email adresses you can virtually mail to if they go home at evening
- day night cycle with shifts and shiftplanner
- IT department and computers that crash with clumpsy supportdesk answering stupid questions to stupid sulutions
- driving Medical COW's

omg.. i could go on for hours... i mean there is so much possible now :) please keep developing/building on the game!!!!

Short answer: Anything you like to do.

Somewhat longer answer:

Most open source projects are not organized much, and people just do what they like to do, for their own reasons.
You don't need permission or so, just start working on something you fancy doing. Ask help when you get stuck, or are unsure how to proceed. (The bottom of http://code.google.com/p/corsix-th/wiki/GettingStarted gives some starting points, asking at IRC also works quite well).

The chance of getting your work integrated into the game gets a lot bigger when you work on the goal of the project. CTH currently aims to be a fully open clone of TH.
If you are going on something big, it never hurts to ask the opinion of a developer first.
